Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I had an appointment in the afternoon for an ultrasound and I decided to go early for a walk-in x-ray i also required as I know it can be very busy. Without asking they were nice enough to reschedule my ultrasound since it wasn't busy to coordinate with my x-rays. The technician who did the ultrasound was lovely. the X-ray person was very professional and efficient. I've waited in the past at this clinic but this past experience was very good.

I had a pelvic ultrasound with a sonographer that her name is Farahnaz Sohrabi. Honestly, I had several times sonography from another parts of my body and always when I call for making an appointment for ultrasound, I ask for her to do my sonography. She is really professional in her field and her diagnosis is always correct. Thank you 🙏🏼

Had super positive experience with the team of professionals at Well Diagnostic Clinic. Very short ( 3-4 minutes) waiting time , reception team members are welcoming and supportive ; the US professional Farah was amazing ! She sorted out the requisition ( which was not exactly what my child had to do so with her guidance we were able to request the correct requisition right on the spot from our doctor ( who was super helpful too) and the rest of the things went smoothly!
