MyHealth Centre is now WELL Health Diagnostic Centre in Sudbury! We’re the same team with a new name, located in suite 107 of the medical building on Lasalle Boulevard between Carmen Street and Attlee Avenue. We provide OHIP-covered ultrasound and X-ray services with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis with a signed requisition**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Unfortunately the office was very busy when I arrived, the office clerk was away from the counter for me to register. When she arrived she addressed me to let me know that the computer system was down and at the time she was unable to register me and would let me know ASAP.
I had a scheduled appointment and was taken in before my scheduled time and treated very well during my ultrasound and xray.
Thank you to all 3 ladies that worked yesterday at the Lasalle location.
