MyHealth Centre is now WELL Health Diagnostic Centre in Scarborough! We’re the same team with a new name, located in the Birchmount Plaza at the corner of Birchmount and Danforth Road, near Dollarama and Pharmasave. We provide OHIP-covered cardiology, ultrasound, X-ray and bone mineral density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I wish I asked who my technicians were today for my 36 week ultrasound but one of the young ladies was in training and did my scan, the other assisted. But they were both fabulous. Would highly recommend going here. Some clinics don’t say a word or they won’t even mention “hey, we will take our photos and then let you see your baby”, some are so silent and that can be worrisome for a new first time mom! But I’m going back there again most definitely! So clean and the technicians were great and informative and calming!

Dr Hussein saved my life , I’m now free from heart desease blockages my arteries , his experience and concern prompted me to do this procedure and thankfully It saved my life , thank you dr and all the staff who took care of me with all the tests that I did , cordial thank you to Nakita who listened to my concerns and followed through .
