MyHealth Centre is now WELL Health Diagnostic Centre in Sudbury! We’re the same team with a new name, located in suite 103 of the Four Corners Medical Arts Centre next to Shoppers Drug Mart. We provide OHIP-covered ultrasound and X-ray services with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis with a signed requisition**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I attended My Health Centre on Long Lk. Road as a walking patient. The waiting room was full and just as I was about to question whether I should come back another time (overwhelmed by the idea of having to wait).

I was greeted with such warmth and kindness. Carrie registered me and was so pleasant. She kept patients updated on their approximate time wait and offered mints, answered any questions people had, and Carrie's vibe/essence made the overall experience seemless and somehow her happy demeanor made the wait seem like seconds in length (reality of my wait time was about 1.5hrs).

The techs were also very professional and pleasant despite the great number of patients they had been serving and still had to serve. Nobody was rushed and no one gave off a sense of " irritated " by their obvious workload.

Fantastic place, fantastic people, and let's hope for fantastic results as well!

The X-ray Tech was kind and very professional. Although the waiting room was busy, I didn’t feel rushed. I was having difficulty breathing and having a hard time changing into the gown. She helped where she could and was very patient and supportive. Although I was feeling unwell, it was actually a positive experience.
