MyHealth Centre is now WELL Health Diagnostic Centre in Sudbury! We’re the same team with a new name, located in suites 402 and 407 of the Larch Medical Building, just east of Durham Street. We provide OHIP-covered cardiology and diagnostic imaging services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I was very pleased with my visit to receive an ultrasound. The wait period was short, staff was courteous and friendly. The facility was also clean which I appreciated!
I was very well welcomed, and what I especially appreciated is that I received reminders 3 days and then 2 days before my appointment. The radiologist who took care of me was very patient and adapted to my pace. Since I am francophone and my English is not very strong yet, she explained things calmly so that I could understand well.

They were very efficient and helpful when i did the booking, and i was given all the info that i needed. Besides the texts they gave me a follow-up phone call two days before. On the appointment day I couldn’t find their entrance (I was in the wrong side of the building), and they gave me excellent directions. The check-in staff and bone-density technician were all extremely friendly and welcoming. A great experience.

Beautiful new office, clean and friendly. There was a fire alarm while I was in between tests and the tech made certain I was okay when we had to evacuate the premises, coat in hand, wearing a gown. We had a good laugh at the situation and once all was checked by the fire department, we (patients, staff) resumed operations. Everyone was friendly and calm. Already got my scans and report in Pocket Health within a day.

Very efficient, very professional, new environment so it's very modern and spotless!
It was awesome. I got there early and they took me right away. Explained everything to me as I had 3 major scans that took up the day but every little detail was explained and staff was extremely friendly and compassionate.
I have had another ultrasound at the new location and they took me immediately and also took my X-ray requisition and added the X-ray at the same time. Very kind and helpful and patient workers. Thank you
