Ontario Imaging Diagnostics is now WELL Health Diagnostic Centres! We’re the same team with a new name located in North York at 20 Wynford Drive, Suite 312. We provide OHIP-covered ultrasound and X-ray services with shorter wait times, faster report turn-around, and the highest standard of patient care. Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Had a great experience here for a walk-in X-ray. The whole process was quick, smooth, and efficient. From the time I arrived to the time I was finished, I was in and out in about 20 minutes! It's always appreciated when a medical appointment doesn't turn into a half-day event. The staff kept things moving, and everything was straightforward without any unnecessary waiting around. The location was convenient, and the quick turnaround made the entire visit stress-free. It's nice to find a diagnostic centre where you can walk in, get what you need done, and get on with the rest of your day. Five stars for efficiency, convenience, and a hassle-free experience. Definitely appreciate how quickly everything was handled!
