MyHealth Centre is now WELL Health Diagnostic Centre in Mississauga! We’re the same team with a new name, located in the Credit Valley Professional Building beside the hospital at the intersection of Erin Mills Parkway and Eglinton Avenue West. We provide OHIP-covered cardiology and diagnostic imaging services with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

My mother had an appointment for a cardiac stress test, and our experience was excellent. There was no waiting time, and both Ann at the front desk and Majid, the technician, were very friendly, kind, and professional. My mother was feeling a little anxious before the test, but they made her feel comfortable and reassured her throughout the process. Everything went smoothly, and we truly appreciated their care and kindness. I would highly recommend this place to my family and friends.
