MyHealth Centre is now WELL Health Diagnostic Centre in Milton! We’re the same team with a new name, located in the Milton Professional Centre on Bronte Street South, just north of Derry Road. We provide OHIP-covered ultrasound, X-ray, mammography and bone mineral density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Great staff and service. I had an issue with my paperwork which I thought was taken care of electronically. Ravia and her team went above and beyond to accommodate me and get my appointment started despite the hiccup. Ravia stayed more than an hour past her working hours and worked relentlessly to trace electronic trails and reschedule other patients appointments to resolve the issue. Thanks to her dilligence and flexibility, I was able to have my time sensitive appointment. Nadia was my technician and was very quick and efficient throughout the process. Overall, the clinic is organized and well maintained. Suggestions to improve would be better integration of the online live chat booking process with the actual patient records to reduce administrative and technological uncertainties. I would definitely book future appointments at this clinic.

Wonderful experience for pregnancy ultrasounds. Kind staff, short wait, they let your partner in, and always printed us pictures. Totally recommend WELL Health over getting diagnostic imaging done at a hospital, as they are both covered by OHIP and the overall experience was much better!

100% recommend any WellHealth Clinic for all diagnostic imaging. Especially if you are doing a pregnancy ultrasound; much better experience than the hospital will give with less wait, kind staff, covered by OHIP, and they always printed pictures (hospital never did this)!

I couldn’t find my requisition form. The receptionist called the Dr. Office to ask them to re-fax the request & as soon as she received it she informed me that the technician will take me for my bone density test as soon as possible & it did not take time (less than 10 min).
