MyHealth Centre is now WELL Health Diagnostic Centre in London! We’re the same team with a new name, located in the North London Medical Centre, just east of SmartCentres shopping plaza. We provide OHIP-covered nuclear cardiology, ultrasound, X-ray, mammography, and bone mineral density – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

My daughter is very timid and very anxious. She hates being in close proximity to anyone let alone for an ultrasound. Plus, my son had to come with us so he was in the exam room as well while she had her abdomen exams. The tech was very patient with both of them. They’re both on ASD so they can be a little difficult (my son talks - a lot, my daughter avoids people and is very nervous). The tech took his time and was supportive of my daughter, enough so that she felt comfortable by the end. He was great. Couldn’t have had a better tech!

The examiner was very thorough and made sure to explain each step of the process clearly, which really helped alleviate any discomfort.
