MyHealth Centre is now WELL Health Diagnostic Centre in London! We’re the same team with a new name, located in the North London Medical Centre, just east of SmartCentres shopping plaza. We provide OHIP-covered nuclear cardiology, ultrasound, X-ray, mammography, and bone mineral density – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Efficient and friendly! Arrived 1515 for a 1535 double appt. I was taken in shortly after checking in for the first appt and 5 minutes after that to the second appt. I was in my car and heading out at 1534, 1 minute before my appts were to start! Love it!!!

This medical center is the newest and the best medical Center in London. First, I want to thank the entire medical and administrative staff in this center, especially the medical staff in the cardiology department. I fainted "because I am a diabetic and a patient with blood pressure" while preparing for the cardiac stress test , where the entire medical staff helped me to regain my consciousness and do the test , and after the completion they did not allow me to leave until after making sure of my health condition. In addition to having the latest medical equipment, its location is great, there is no traffic congestion and there is a large car park for the building only. Also, I would tell the new patients "as patients without reservation" that the parking is free, but you have to take the parking card for the reception staff until they give you the exit ticke

I had a Nuclear Stress test here. I had little idea of what to expect. Everyone, from reception to the end of the second appointment were professional, kind, knowledgeable and skilled. I had 3 wonderful women involved in my care but don’t remember their names. (Sorry). John was the nuclear technician that I saw both days. He put me at ease (as did the ladies) quickly and I knew I was in safe hands. Thank you so much.
