Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Alberta Health Card (AHCIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our AHCIP-covered services require a signed requisition from a certified Alberta healthcare provider, such as a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Alberta healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

The doctor was very professional and yet, very interested in what I had to say. She took the time to listen to my concerns. She held a very positive position in how my treatment was coming along, ( although I felt I was doing poorly). She left me feeling good about myself. Recommended things to try through some of my rough patches. It's quite a satisfying feeling when a medical professional actually takes the time to listen to you, and not throw a prescription at you and rush you out the door...
