Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail
or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

As soon as we enter the centre the staffs are very welcoming. Mr. Quamrul is a very professional staff. He leads me to my room. I requested more pillows and additional blanket which he immediately gave me. He explained everything what's gonna happen as well as how to fill up the form with questionnaire. Mr. Quamrul spoke so gentle, and placed all the electrodes, he is very experienced and knows the job well. He makes my night comfortable. Overnight I slept very well. I very much recommend this diagnostic centre, especially the professionalism of Mr. Quamrul. Thank you for taking care of me.
