Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Alberta Health Card (AHCIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our AHCIP-covered services require a signed requisition from a certified Alberta healthcare provider, such as a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Alberta healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Dr. Sandercock was an exceptional physician. Very kind.
However her receptionist almost scolded me gor not checking in. Upon arrival she was not there. So I checked in with the other desk. They said she would be out soon. Assuming she meant Dr. Sandercock. When the receptionist arrived. She sat for awhile. I was the only one in the waiting room. Then finally asked if I was for Dr. Sandercock. I said yes. She proceeded to say I needed to check in and the other desk was for something else. How is a patient to know they don’t communicate or are for different things. She was quite abrupt and said she wasn’t sure if the would see me as I was sneezing. I had a mask on the whole time. I have allergies and was congested.
