MyHealth Centre is now WELL Health Diagnostic Centre in Sudbury! We’re the same team with a new name, and our radiology services at Larch Medical Building have moved to this new and improved facility in Elm Place, suite 255. We provide OHIP-covered nuclear cardiology, ultrasound, mammography, and bone density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

This was my first mammogram and the first time visiting the new location in the mall. The direction for parking were easy, the signage inside the mall was everywhere and it was easy to find where I was suppose to go, the intake clerk was helpful and best of all my technician for exam was A+. I was nervous , being my first time, but she took great care and was professional
