MyHealth Centre is now WELL Health Diagnostic Centre in Milton! We’re the same team with a new name, located in the Milton Professional Centre on Bronte Street South, just north of Derry Road. We provide OHIP-covered ultrasound, X-ray, mammography and bone mineral density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I had my first ultrasound appointment for my baby here and it was a great experience overall. I travelled on a day of a storm and the staff were very accommodating as I was coming from London. My ultrasound tech was professional and he was very knowledgeable and provided me various ultrasound pictures. Overall great experience.

Very professional front line staff. Easy to book an appointment. Efficient and friendly processing of documents upon arrival. As an unexpected walkin patient i was prepared to nook an appointment gor an exray and also a followup appointment in 6 months for an ultrasound as requested by my doctor. Imagine my surprise when they offered to take me right away for my exray which happened within 5 minutes of my arrival.

The exray technician was very courteous and professional. His instructions were clear, friendly and caring. I was in and out within 30 minutes.

In today's government assisted medical care environment i am absolutely pleased and greatly impressed with the staff, facilities and performance of the Well Diagnostic Clinic in Milton.

I was pleasantly pleased with the care and tests the staff provided at Well Health Diagnostic Centre in Milton. The young lady at the front desk, and all the technicians in the Cardiology Department were all wonderful. Anne was very friendly and had me laughing which helped me feel relaxed.
For each of my 3 visits to the clinic, Kalene was very friendly, always smiling and extremely patient. She provided a thorough explanation of every step in the process, to keep my mind at ease.
I didn't get the names of the other technicians, but they were all wonderful!

I arrived for my 2 day nuclear cardiology stress test early and was called into the clinic well before my scheduled appointment which I appreciated. Anne the prep nurse was very friendly and highly competent. Kaitlyn the stress test nurse was equally friendly and demonstrated high levels of competence as well. As did the nuclear tech whose name I don’t remember. All in all your people were well trained professionals and a delight to interact with.
