Step 1: Requisition

Download a requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Alberta Health Card (AHCIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our AHCIP-covered services require a signed requisition from a certified Alberta healthcare provider, such as a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Alberta healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Such a great place. Finally feel like someone is listening to my issues and trying to help. As a menopausal woman it seems like many doctors dismiss the symptoms. This place actually listens to women and all the issues, the lack of estrogen, progesterone, testosterone and weight gain that are caused due to Hormonal imbalance as you go through your later years. If you need help go here great experience with this place.
