Step 1: Requisition

Download a requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

She treated me like a friend more than a patient which I was really appreciative of, it made the entire experience way more enjoyable to have someone working who loves their job. I asked the person her name so I could send a personal, heartfelt thank you. Of course in all my emotion at the time, I have forgotten it. I hope she sees this. This person very much exceeded my hopes and expectations. With her ultrasound training and experience, I think she found the problem. I know she couldn't tell me that she found it, but she found where the pain is while performing the procedure. The outcome might not be pleasant, but she found the pain. I believe she has me headed in the right direction for proper care and hopefully recovery. I'm grateful my appointment was with her in Delhi.
