We provide AHCIP-covered endocrinology, diabetes, diet and nutrition services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Alberta Health Card (AHCIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our AHCIP-covered services require a signed requisition from a certified Alberta healthcare provider, such as a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Alberta healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

When ever I have gone to C-Endo now Well Health I’m always treated well and the staff are friendly and great to deal with. The wait to see my endocrinologist is short and when I see him he is genuinely concerned for my health and asks the right questions and I feel my needs for my type 2 Diabetes is always of top priority.
