MyHealth Centre is now WELL Health Diagnostic Centre in Brampton! We’re the same team with a new name, located in the Chrysler Drive Centre at the corner of Williams Parkway and Chrysler Drive. We provide OHIP-covered nuclear cardiology with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail
or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

From the front reception to Michael who did my tests, and meeting with Dr Jurt. All staff were warm caring kind and patient. This is my second experience with this team. Dr Jurt took the time to respond to all my questions, concerns and next steps. In addition I appreciated her recommendations and next steps. I had a great experience at Well Health Diagnostic Centre- Chrysler Drive in Brampton. I was impressed in the waiting area with the sign posted on the chairs that they were sanitized and the area very cleaned. The staff are all friendly and professional. The technicians walk me through every procedure that was being done to me which makes me more comfortable.
The doctor( sorry I forgot his name- African Canadian) was very gentle and caring. My two days experience was amazing 👏.

It was great getting my heart diagnostics done so fast at this privately owned lab, I i was treated very well and professionally by the staff, all of whom were foreign trained. I only wish our federal government would open the border to more, skilled people like this. oHIP paid for everything and I didn't even have to pay for parking, right outside their front door!!!
