MyHealth Centre is now WELL Health Diagnostic Centre in midtown Toronto! We’re the same team with a new name, located in the Davisville Medical Dental Centre on the east side of Yonge Street just south of Davisville Avenue. We provide OHIP-covered cardiology services with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I had a really great experience at this clinic. The receptionist was incredibly kind, warm, and welcoming. She took me seriously right away when I expressed concerns about my heart, which meant a lot to me and helped ease my anxiety.

I arrived a few minutes early for my 2:45 PM appointment on January 6, 2024 and within minutes was in a room doing an echocardiogram. The clinician was very professional and communicated well. The clinic was very busy so I did have to wait. About 40 minutes to do the second assessment which was a treadmill test. Again, the clinician was extremely professional and communicated well. I would happily recommend this clinic to anyone seeking medical assessments.
