MyHealth Centre is now WELL Health Diagnostic Centre in Mississauga! We’re the same team with a new name, located in the Credit Valley Professional Building beside the hospital at the intersection of Erin Mills Parkway and Eglinton Avenue West. We provide OHIP-covered ultrasound, X-ray, mammography and bone mineral density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Had a mammogram at credit valley last week. the technologists name was Jennifer, she is so good at what she does. Quick and with as little discomfort as possible.
As luck would have it, this was the second mammogram I’ve had done by her. Based on many of the experiences I’ve had through the years I think she should be training all of the mammogram technologist - these shouldn’t be something we dread going to.
