We provide OHIP-covered cardiology services with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Other than the need to pay for parking, this experience was excellent. Negotiated an appointment day/time that worked with my schedule. Was called in at that time. The woman who conducted the test did a great job with communication and was very professional and kind. Would definitely recommend this clinic for a bone density test.

I found the staff very professional, friendly and warm. The lady is the mammogram suite was especially wonderful at her job and put me at ease immediately. However, I did find the clinic itself a little dirty from fingermarks on the cover screen to the old torn and ripped gown provided to wear prior to my procedure. Your people excell but the physical space of the clinic needs more attention throughout the day.
