MyHealth Partners Inc. dba WELL Health Diagnostic Centres (“WELL Diagnostics”, “we” or “our”) and our parent company WELL Health Technologies Corp. (“WELL”) are proud to offer you the WELL Diagnostics website (the “Site”) and the associated clinics in Ontario and Alberta (“Clinics”).

When you submit a form on the Site, we collect personal information such as your name, email, phone number, address, and health card number (and, for healthcare professionals, billing or regulatory college number). This information is used to create a profile, verify your identity, respond to inquiries, or follow up with you.

If you are a physician submitting an inquiry into the Healthcare Provider Portal, we collect your name, practice location, email, billing number, and regulatory college number to verify your identity or create your Portal account.

Direct Marketing

If you sign up for direct marketing or promotional communications from WELL Diagnostics or WELL, we collect your name and email to inform you about products, services, and offers.

Opportunities for Physicians

If you are a Canadian physician filling out our online form for employment or partnership opportunities, we collect your name, email, phone number, and other relevant details to allow our team to contact you.

WELL Diagnostics Clinics

At WELL Diagnostics Clinics, we collect personal information such as your name, email, phone number, address, and health card number to create a profile, verify identity, communicate with you, and bill insurers for services provided. We may also collect personal health information including health history, conditions, and treatments directly from you or other healthcare professionals.

2. Why do we use personal information?

We use your personal information to:

  • Provide you medical services
  • Review diagnoses and evaluate treatment
  • Determine suitability for research programs or trials
  • Manage our relationship with you and provide requested information
  • Conduct research on our Site and at our Clinics
  • Communicate about inquiries, appointments, or opportunities
  • Detect, prevent, or investigate security breaches
  • Process insurance or payment information

  • Book appointments as per physician orders

WELL Diagnostics Site: Shared only with service providers for site operation and optimization.

WELL Diagnostics Clinics: Shared with healthcare providers or insurers for referrals, trials, billing, or accessing imaging reports.

4. How long do we keep personal information?

We retain personal information for as long as required to provide services or as legally mandated.

Retention periods:

  • Alberta: 10 years (or 2 years after the patient reaches the age of majority)
  • Ontario: 10 years (or 10 years after the patient reaches the age of majority)

5. How do we keep personal information accurate?

We take reasonable steps to ensure accuracy and rely on you to notify us of updates to your personal information.

6. How do we protect your personal information?

We use physical, administrative, and technical safeguards to protect your data. Only authorized staff and service providers with legitimate purposes have access. Security training is provided to all WELL Diagnostics staff.

All vendor contracts include confidentiality and privacy protection clauses. Despite safeguards, no system is completely secure, and breaches cannot be fully eliminated.

7. Where do we store personal information?

WELL Diagnostics Site: Data collected via secure connections and stored on secure Canadian servers. Some third-party processing may occur outside Canada, where local laws may permit government access.

WELL Diagnostics Clinics: All clinic data is stored within secure Canadian EMR systems. Vendors comply with Canadian privacy laws and data protection standards.

8. Links to third-party sites

Our Site may link to external websites that are not affiliated with WELL. These third parties have their own privacy policies. We are not responsible for how they collect or use personal information.

9. Direct marketing

Where you consent, we may use your information for marketing communications regarding WELL Diagnostics and WELL affiliates.

You can opt out at any time by:

10. Use of Technology including Artificial Intelligence

WELL Diagnostics and WELL use digital tools, including artificial intelligence (AI), to enhance patient care, streamline processes, and improve clinic experiences.

  • Electronic Medical Record (EMR) Software: Securely stores and manages patient health data.
  • Online Booking System: Allows patients to book, modify, or cancel appointments securely.
  • Automated Fax Triage: Sorts and prioritizes medical documents for faster processing.
  • Ambient Scribe Software: AI-driven transcription of patient interactions reviewed by physicians.
  • Clinical Decision Support Software: Uses machine learning to help identify chronic and rare diseases early.

These tools enhance data accuracy, provider collaboration, and patient outcomes.

11. Consent

We obtain patient consent to collect, use, or disclose information except where otherwise permitted by law. Consent may be oral, written, electronic, or implied when obvious from the context.

Physician consent is also obtained for use of their information in service delivery.

Depending on your province, you may have the right to:

  • Request data transfer to another organization
